Camp Forms

ALL forms are due two weeks before your campers are scheduled to attend camp.

Mandatory Medical Forms that MUST be Completed

A copy of your most recent physical exam and immunization record from your Physician's office.

The Parent Information Packet contains many of the nuts and bolts to help parents and campers prepare for camp.
