Camp Forms

ALL forms are due two weeks before your campers are scheduled to attend camp.

Required Forms (Due 2 Weeks Before Your Session)

All forms must be uploaded to your Active Account. Campers cannot participate without these:

  1. Most Recent Physical Exam + Immunization Records

  2. Sunscreen & Insect Repellent Authorization

  3. Ropes Course Waiver
